Individual $125/month, couple $200, family (up to four) $275, for each additional child up to age 21 $20 per month, individual (one time) visit $145 with one follow up in 30 days.

Individual Membership

$125 per month

If choosing to pay for a year in full there is a 10% discount- price is then $1350 for the year.

This membership offers personalized, comprehensive care for primary care as well as functional medicine. At Functional Now we use Dr. Chrono electronic health record system which includes a user friendly patient portal called OnPatient. This patient portal allows you to access your after visit summary, communicate with your provider, request refills, and manage your care by scheduling and canceling appointments.

Couple Membership (Two Individuals)

$200 per month

If choosing to pay for a year in full there is a 10% discount- price is then $2160 for the year.

Family Membership (up to 4 people two adults and two children- children up to age 21)

$275 per month

If choosing to pay for a year in full there is a 10% discount- price is then $2970 for the year. Additional children up to the age of 21 may be added on at an additional $20 per month per child.

Non-Member Visit

$145

This is an initial visit with a follow up within 30 days of initial visit. Limited to once per year if additional visits are needed membership will be required. If the patient decides to sign up for the membership after the individual visit the $145 will be applied to their membership.
